Northern Terminus: Entrance to Grand Beach Provincial Park.
 
Southern Terminus: Canada/U.S. border between Middlebro, MB and Warroad, MN.
PTH 12 travels north-south from Grand Beach south through Beausejour and Steinbach, ending at the U.S. border near Lake of the Woods.
PTH 12 was one of the original routes when the trunk highway system was first introduced and as late as the 1950's, it followed the route of the old Dawson Trail from Winnipeg to Ste. Anne. That portion of the route largely comprises present-day PR 207 and PTH 12 was re-routed to connect with PTH 22 that ran between Beausejour and Grand Beach. The route was also extended south to Steinbach and east to the extreme southeast corner of the province. Click here for images of the former routing of PTH 12.
The portion of the present-day route between PTH 1 and the U.S. border is part of Mom's Way, a route from Winnipeg to Thunder Bay that travels through Minnesota that was once a route used by Aboriginal peoples prior to North America's settlement by European immigrants.
